As part of our commitment to serving our parish family, the Annunciation Cathedral is pleased to provide parishioners and families in good standing with official copies of sacramental records for Baptisms, Chrismations, Marriages, and Funerals celebrated at the Cathedral. Before submitting your request, please review the information below. Once you are ready, complete the request form at the bottom of the page. After your request has been received and verified, a member of our office will contact you with the next steps.

Before You Begin

To help us locate your record efficiently, you will be asked to provide:

Full name(s) associated with the sacrament
Date of birth (if applicable)
Type of sacrament requested
Approximate date of the sacrament
Names of parents, sponsors, or spouse (depending on the sacrament)
Any additional details that may assist our records team

You will also have the option to upload supporting documentation if available.

Additional Information

Only records on file at Annunciation Cathedral can be fulfilled
Identification or verification may be required for certain records

Stewardship

As part of our ministry to our parish family, official sacramental records are provided as a stewardship benefit to parishioners and families in good standing who are current Stewards of the Annunciation Cathedral. If you are not currently a Steward, we invite you to become one and join us in supporting the ministries and mission of our Cathedral. Once your stewardship commitment is established, we will be happy to assist you with your sacramental record request.
